CONTINUING CARE

Dear Valued Patient, If you are currently on an ongoing therapy — including GLP-1, or other prescribed protocols, please establish care with another licensed provider promptly so your treatment can be reviewed and either continued or safely discontinued.

Do not stop or change any prescribed treatment on your own. If you have questions before you can be seen, contact your primary care provider or a local urgent care clinic.

YOUR MEDICAL RECORDS

Your records are being securely retained in accordance with Arizona law (A.R.S. § 12-2297): at least six years from your last date of service for adult patients.

To request a copy, email Info@vipivwellness.com We will send you a records release authorization form. For your protection, we verify identity and require a signed authorization before releasing any records — we cannot send records to an email address without one. There is no charge.

We encourage you to submit requests by 9/11/2026 so they can be processed promptly, though your right to request records continues throughout the retention period above.
